This is to inform you that the Application forms for the verification of examination marks & grades can be downloaded from Faculty website and required to deposit an Recorrection fee (Rs.500/= per one subject) to Bank of Ceylon, Kuliyapitiya – A/C No. 0001473508 and scanned copy of the filled application form & deposit slip should be emailed to rappliedsciences@gmail.com on or before 19.02.2021. (Please note that, payments made to Cash Deposit Machines will not be considered. You must make the payment to the bank counter. You must state your name and the registration number in the deposit slip, before making the deposit.)
Hard copies of the filled applications & payment slips should be posted to the following addres.
